Payment is expected at time of service. We accept major credit / debit cards, cash and checks (with a copy of your valid local driver's license). For patients with insurance benefits, you may be able to submit a claim to your insurance provider for your office visit and any procedures or labs that are done at our clinic, but this is your responsibility. Additionally, you may be able to use insurance benefits for prescribed medications and lab work. Insurance providers may require you to go to a specific lab or pharmacy (such as Quest or LabCorp, CVS or Walgreens). Please avail yourself of this information so that you may indicate your chosen lab or pharmacy. In the event a check is refused / returned for any reason, you agree to an additional $49 returned check fee along with any and all costs associated with debt collection, including attorney's fees. Trinity Men’s Health reserves the right to refuse payment by check for any reason.

If warranted, and at the sole discretion of the physician, we administer treatment. With respect to testosterone replacement, we do not prescribe oral or transdermal preparations due to their possible side effect profiles and potential for abuse.
